Berkshires Operational Enhancements to Reduce Re-Admissions
This TA project for Health Collaborative of the Berkshires in partnership with Fallon Community Health Plan helped them implement operational improvements to reduce inpatient re-admissions and streamline the inpatient assessment and discharge process. As part of this project, Health Management Associates developed a document of project highlights including objectives, goals, recommendations, references, example workflows, and case management tools.
