Care Coordination Workforce Development Project
This TA project for Family Service Association delivered care coordination training for LTSS CP care coordinators. As part of this TA project, Advocates for Human Potential also delivered a report of observations and recommendations for operational policy and practice changes and an Action Plan for care coordinator training.
