Quality/Performance Improvement (Planning Phase) – Establishing Data-Driven QI Intervention Plan
Achieving a high-quality score/performance requires a detailed measure-performance plan that is data-driven. The plan must take into account data on the baseline performance of each measure against the human and technical capabilities of the entity to determine priorities and actions that will maximize the overall performance of the quality program.
Medicalincs offers clinical analytics and informatics support to ACOs and CPs to develop an annual quality performance improvement plan. Medicalincs will work with your quality team to:
PART 1. Establish Baseline Performance: Analyze data sources and data for each measure and glean insights from performance trends.
PART 2. Prioritize Measures for Action: Based on data trends and on organizational technical and human capital capabilities, prioritize measures for action.
PART 3. Identify Measure-Improvement Strategies: Develop a set of strategies for improving the performance of all prioritized measures, taking into account evidence-based solutions and best-practices, and organizational strength.
Medicalincs is prepared to provide TA for developing the plan to a single entity or to a consortium of entities as a hands-on learning collaborating. The latter option may apply to several providers within an ACO.
