Care Collaboratives and Referral Pathways are an important part of advancing primary care as well as comprehensive patient care coordination. We understand there are critical components within a care collaborative that will ultimately affect the patients’ experience in terms of access, communication, transition of care, and closing referral loops. Medicalincs has firsthand experience in strategies to establish care collaboratives. As part of the Collaborative Care and Referral Pathways project Medicalincs subject matter experts will offer TA assistance with the following:

• Reviewing data to help identify and recommend which providers to target for care collaboratives
• Establishing a process/workflow to develop & monitor care collaboratives and referral pathways
• Developing care collaborative documents and templates for ongoing needs
