COACH: Getting out of the “fix-it” framework through meaningful engagement
COACH is a framework of techniques and tools designed to engage and empower individuals with complex health and social needs. Care team members will learn behaviors required to build authentic healing relationships, build self-efficacy, and create sustained behavioral change. The COACH training trains up to 50 care team members how to break down relationship building and empowerment into actionable behaviors.
Partnered with COACH, RELATE trains up to 10 supervisory staff to promote complex care staff adaptability, flexibility, and positive team dynamics. RELATE helps supervisors to build resilience in frontline staff to better tolerate the work’s emotional impact, and then to innovate and think creatively to address challenges.
COACH & RELATE Kick off Trainings: Our COACH training is an interactive series of training events delivered virtually via Zoom (due to COVID-19 pandemic). COACH training is offered to both front line staff and supervisors and will occur in five approximately two and a half hour virtual sessions. The RELATE training is also delivered virtually via Zoom (due to COVID-19 pandemic), and is offered to supervisors only, and will occur in one three-hour virtual session.
Mid-point Trainings: Three-hour sessions reinforce learning and provides momentum for the last six months of implementation. The COACH session includes themes captured in case conferencing and coaching calls. The RELATE session includes relevant themes captured in on-going RELATE coaching calls.
On-going coaching is offered to front line staff and supervisors to continue to build on skills and knowledge base within the COACH/RELATE model, and other complex care skills and engagement strategies. This additional coaching is offered via monthly, hour-long COACH and RELATE case conference calls, and up to six Skills Labs. Camden Coalition also offers monthly systems calls with organizational leadership to help guide efforts in COACH/RELATE implementation across the organization.
COACH Case Conferencing Call: Group meeting focused on case questions, challenges, and continued guidance on the application of the COACH model in practice. Case workers will be required to fill out case conferencing scripts ahead of each call.
RELATE Coaching Call: Continued training on the RELATE model to ensure that supervisory staff have technical expertise to manage patient-facing staff.
Skills Labs: Supplemental mini trainings focused on core tenets of complex care delivery, allowing frontline staff to practice complex care skills and engagement strategies, via virtual educational instruction and experiential learning.
Monthly Systems Calls: Camden Coalition subject matter experts will join leadership personnel on a monthly system call to discuss program design, identify needs and themes, and monitor and strategize about the overall progress of COACH and RELATE. These calls will occur via telephone or Zoom and include designated leadership personnel and at least two Camden SMEs.
