Addressing Food Insufficiency for Individuals with Chronic Illness
Health Policy Matters (HPM) has expert knowledge of Flexible Services for nutrition with a focus on both Medically Tailored Meals (MTM) for high risk members and other cost-effective options for ACO members at low or moderate risk.
The goal of this project is to support effective and efficient implementation of nutrition supports for ACO members. The project can be achieved in collaboration with Behavioral Health CPs if desired.
The objectives of the project are to:
- Assist in key implementation issues for nutrition flexible services. This may include different types of services for members at different levels of risk including, but not limited to, evaluating options to support members during the COVID-19 pandemic;
- Develop efficient processes to screen, enroll and track the delivery of nutrition-related Flexible Services;
- Develop and measure quality metrics for nutrition-related Flexible Services;
- Report to the State on the administration of nutrition-related Flexible Services.
HPM will work with your ACO to achieve all of these steps as part of the process of delivering Flexible Services that support nutritional needs.
