Reduce Medicaid Readmissions Using ASPIRE – Training
Reducing readmissions is a key competency and pay-for-performance metric for ACOs, and improving follow up after hospitalization is a quality metric for CPs. It’s not easy, but it is possible – and many organizations have implemented effective strategies that you can implement as well! We wrote the book on reducing Medicaid readmissions: the AHRQ Hospital Guide to Reducing Medicaid Readmissions (known as the ASPIRE Guide). We possess nationally-recognized and unique expertise in designing and delivering whole-person transitional care to measurably reduce Medicaid readmissions. This engagement will provide education on the ASPIRE guide, through an 8-part webinar series with Q&A that will advance your teams’ understanding of how to “Design and Deliver Whole-Person Transitional Care.”
CHS is happy to customize this TA opportunity in any of a variety of ways. This could certainly lend itself to ongoing strategic and technical advisement and performance improvement coaching. Our experience providing monthly coaching is that teaching is just the beginning of an effective strategy: success in “bending the trend” of readmissions requires a trusted coach, an operational dashboard, and regular use of the Plan Do Study Act method (small scale tests of change) to improve “how” we do “what” we do to achieve outcomes. We always look to serve at the largest scale possible, and this offering would be great to customize for collaborations with multiple ACOs and/or CPs.
