Planning and Pricing Care Coordination and Outreach Services
As DSRIP investment dollars decrease over time, ACOs and CPs are exploring value-based financial and contracting arrangements to support elements of care coordination and outreach services, particularly for high-risk behavioral health and LTSS populations. This offering provides 75 hours of TA to an ACO or CP to support planning and pricing of care coordination and outreach activities. HMA will work with your organization’s leadership to identify its strategic and operational considerations with respect to buying or selling care coordination and outreach services. HMA will then assist your organization in building one or more Microsoft Excel-based pricing models (potentially leveraging an HMA “service model” pricing template as applicable) to estimate fixed and variable administrative and program costs related to the provision of care coordination and outreach services. The service model will be built to estimate “break even” points and identify key assumptions and risks that will drive costs and revenue. HMA can customize the scope to more specifically address an ACO or CP’s unique needs. Additional TA Hours: Customization is available with respect to number of TA hours an organization requests to support the project. Total requested hours will influence pricing. Onsite Participation: TA support will be provided telephonically or via online audio/video technology unless otherwise requested. Onsite HMA participation will influence pricing.
