Long-Term Care Management (Including for Special Needs Population)
Comprehensive Care management means working with and for beneficiaries, generally outside of face-to-face primary care office visits, to help them meet their goals. Care management can be a resource-intensive intervention. Therefore, using a dynamic process such as Composite Risk Stratification (that takes into account Social Determinants of Health) that assigns a risk status to each beneficiary helps to direct targeted care management resources, especially for beneficiaries with complex needs.
Medicalincs offers care management and care coordination support to handle the care of high risk, high need beneficiaries, including special needs children. We ensure that our care management support is integrated with the operations and processes of our partner ACO and CP. Our efforts include training on risk stratification, being part of an interdisciplinary team, aid in health screenings for beneficiaries, and care management best practices. We can also remotely support with establishing risk stratification methodologies and some care coordination support.
