Goal: The goal of this project is to enhance BH CP and Adult Community Clinical Services (ACCS) integration by implementing guidance on strategies to do so.

Objectives:

• Create a culture of collaboration between the two programs, including tangible and agreed upon strategies to integrate care and service delivery
• Identify and evaluate strategies to increase coordination between the two programs in a manner that fits within your current operations for both programs
• Create detailed workflows that staff agree to that would result in enhanced integration
• Develop policies and procedures that support integrated care across the BH CP and ACCS programs

HPM will work with your BH CP, leveraging lessons learned performing this work in other BH CP organizations, leaving you with a program that offers greater coordination and better outcomes of care for your patients.

The project would be customized based on the specific infrastructure and needs of the BH CP.
