Creating Housing Opportunities & Sustaining Housing

This Learning Collaborative opportunity will provide ACOs and CPs with an understanding of housing resources available across the state and in local communities that provide opportunities to respond to the housing needs of homeless and housing insecure members. Participants will learn about eligibility and referral processes and the basics on how to navigate and access these resources on behalf of their members. Key housing partners with whom ACOs and CPs may want to pursue partnerships will also be identified and explored, along with strategies and considerations for pursuing partnerships with specific types of housing entities. ACOs/CPS involved in the design and delivery of flexible housing support services can also learn about key principles and interventions that successfully support members to sustain housing. Topics may be customized and participants may be “tracked” based on their particular interests and needs.
