Collaborative Care Management Integration
SMC Partners offers program and process design, data management and systems integration services to enable ACOs and Community Partners to connect their systems and create a community-wide view of care management performance metrics. Today many ACOs and CPs rely on faxes and manually maintained spreadsheets to record events such as care needs screenings, comprehensive assessments, referrals, member outreach/consent, care plan development and sign-off, flexible services, authorizations and invoicing, and member outcomes tracking. These data must be tracked to measure program performance and report to MassHealth. SMC works with ACOs and CPs to develop strategies, processes and technology to capture required information electronically, eliminate faxes and spreadsheets, and show collaborators the full member story.
